Résumé

Objectif : Notre objectif était de construire, à partir du Programme de médicalisation des systèmes d’information (PMSI), un indicateur de suivi des délais d’initiation de la chimiothérapie (CT) adjuvante pour cancer du côlon et de l’évaluer.

Matériel et méthodes : À partir du PMSI au CHU de Rouen, nous avons calculé le délai entre la colectomie pour cancer du côlon et la CT, chez des patients hospitalisés entre 2009 et 2015. Par comparaison au dossier médical, nous avons identifié les vrais et les faux positifs, et nous avons calculé la valeur prédictive positive (VPP) de notre méthode.

Résultats : Parmi les 102 patients sélectionnés, la VPP était de 91 % (IC 95 % : [86–97]).

Conclusion : La qualité de l’indicateur mériterait d’être confirmée dans d’autres établissements. Appliqué aux bases PMSI nationales, il pourrait servir d’outil de pilotage au sein des agences régionales de santé et de l’Institut national du cancer.

Abstract

Aims: Our objective was to construct and then evaluate a monitoring indicator of the initiation time to adjuvant chemotherapy for colon cancer, based on the French diagnosis-related group-based information system (PMSI).

Procedure: From PMSI databases at Rouen University Hospital, we have calculated the time between colectomy and the first chemotherapy in patients hospitalized between 2009 and 2015. We have calculated the positive predictive value (PPV) of our method in comparison with the medical records.

Results: Of the 102 patients selected, the PPV was 91% (95% CI: [86–97]).

Conclusion: The performance of the indicator should be confirmed in other hospital centers. It could be applied to national PMSI databases, and thus be used as a guidance tool within, regional health agencies and the French National Cancer Institute.
